PkDose employs the method of Devine to calculate ideal body weight. Since this formula requires a minimum height of 60 inches, PkDose will accept a lesser entry but will perform IBW calculations as if the patient were 60 inches tall. Note also that ideal body weight is used in the calculation of creatinine clearance which may be, therefore, overstated in patients less than 60 inches tall.

PkDose uses the Cockcroft-Gault equation to calculate the creatinine clearance of adults 21 years of age, or older. That is, Creatinine Clearance = ((140-Age) * Ideal Body Weight)/(72 * Serum Creatinine). For females, the result is multiplied times 0.85.
For children ages 2 through 12 and females ages 13 to 21, Creatinine Clearance = (0.55 * Height in cm)/Serum Creatinine.
For males ages 13 to 21, Creatinine Clearance = (0.7 * Height in cm)/Serum Creatinine.
For children less than 2 years old, Creatinine Clearance = (0.45 * Height in cm)/Serum Creatinine.
Ideal Body Weight has been calculated for a minimum age of 1 year.

Blood draws for therapeutic serum assay rarely coincide with the time of occurence of true peak serum concentrations. Therefore, to aid the clinician in the decision process, PkDose calculates the true peak serum concentration achieved by the sample dose.
While a True Peak serum concentration may be calculated for a loading dose, since the time of administration of the dose is known, the calculation of the True Trough concentration is impossible as the dosing interval has not yet been selected.
If the loading dose were the only dose given, the True Trough would be zero after seven half-lives.
Consequently, the measured Trough concentration appears in the True Trough field and a calculated Trough Serum Concentration is presented upon the clinician's selection of a suggested regimen.
